Wallplate Identification

My signal was cutting in and out and the light on the wall plate flickers. I pulled the wall plate and it looks like something overheated next to the + connector. Where the board is burned the other side has a small SMT with a label that looks like 2X2 (one 2 upside down). How do I identify the board for replacement? I don't see any numbers that give me any clue. All the plate says is 'Winegard'. TIA

How many coax jacks are on the board? Is there a 12 volt socket on the front to plug a 12 volt tv into it? Is there an additional jack on the front of the wallplate?
